Business Situation & Requirements
On average, patients wait up to 24 days to schedule appointments with physicians, a figure that varies by city and specialty, leading to delayed care and poor patient experience. On the provider side, the system grapples with critical issues such as physician burnout, inefficiencies of the Fee-for-Service (FFS) payment model, and frequent patient no-shows, all of which strain healthcare delivery and sustainability.
To address these systemic challenges, VivoDoc envisioned a digital-first platform that could streamline appointment scheduling, reduce wait times, and enable two-way interactions. To transform this vision into a scalable, market-ready solution, the client sought a trusted technology partner with expertise across strategy, product architecture, and rapid time-to-market execution. Solution
After understanding VivoDoc’s product vision and functional requirements, Team Unthinkable proposed a comprehensive development roadmap focused on scalability, performance, and faster time-to-market. The roadmap covered recommendations for a cloud-native architecture using relevant managed cloud services, an optimal technology stack, secure third-party integrations, and compliance with healthcare security standards. Following best development practices, Unthinkable built a unified web and mobile solution using React Native, Angular, and Node.js, designed to significantly reduce patient–physician appointment booking time. The platform was structured to support four distinct user roles: Patient, Physician, Admin, and Super Admin, each with role-specific capabilities to ensure seamless platform operations.
The patient-facing application enables users to discover nearby physicians, view real-time appointment availability, access verified patient reviews, and schedule consultations either in person or via video calls across multiple specialties. For physicians and providers, the platform simplifies onboarding across multiple practices and connects them with potential patients, improving practice visibility and operational efficiency. Admin and Super Admin users are equipped with a centralized dashboard that offers a bird’s-eye view of platform activity, including appointment bookings, onboarded practices, registered providers, and subscription plans, enabling effective monitoring and decision-making across the ecosystem.

As the platform was expected to grow in complexity with evolving features, Unthinkable adopted a modular architecture to ensure scalability and fault isolation. The application was divided into small, independently deployable modules, reducing overall codebase complexity and improving maintainability. This approach enabled faster development cycles, simplified testing, and allowed frequent updates without impacting the entire system, ensuring long-term flexibility and seamless scalability as platform usage increased.
Security Compliance
Given that the platform handles sensitive patient health information (PHI), security and compliance were treated as top priorities. Unthinkable ensured adherence to HIPAA compliance standards to safeguard data against unauthorized access and breaches. Robust security measures were implemented across the application, including secure data handling, controlled access, and compliance-driven workflows, ensuring patient trust while meeting stringent healthcare regulatory requirements across the platform.
EHR Integration
To support seamless electronic health record management and data interoperability, the platform was integrated with leading third-party EHR systems, including Athena Health and DrChrono. Unthinkable addressed integration challenges such as complex onboarding workflows, API inconsistencies, and practice- and provider-level data synchronization. This enabled physicians and providers to access and manage patient records efficiently within their existing workflows, reducing administrative effort and improving care delivery.
Powerful Search Engine
The application features a robust search and analytics engine designed to deliver fast and accurate results for patients. Multiple permutations and combinations were applied to refine search relevance, enabling users to quickly find the most suitable physicians based on location, availability, and specialty. The system was optimized to maintain performance and accuracy even under high traffic conditions, ensuring a smooth and responsive search experience at all times.
Visibility of Available Slots
To ensure patients always have access to the most up-to-date physician availability, Unthinkable implemented an intelligent caching mechanism. This allowed real-time visibility of recently updated appointment slots while minimizing system latency. By optimizing slot synchronization and reducing response time, the solution ensured accurate scheduling information, eliminated booking conflicts, and improved the overall appointment booking experience for patients.
Insurance Verification
The platform enables patients to verify insurance compatibility with physicians or providers during the booking process. This functionality was implemented using a reliable third-party insurance verification service at the backend. By confirming coverage in advance, the solution reduces billing uncertainties, prevents reimbursement issues for providers, and helps patients make informed decisions, resulting in greater transparency and smoother appointment workflows.
Payment to Practitioner
Unthinkable researched and implemented a secure payment mechanism that enables direct payments to physicians or providers. Using Stripe’s Express service, the platform ensures seamless, compliant, and transparent payment processing. This approach simplifies financial transactions, reduces manual intervention, and supports faster settlements, improving trust and efficiency for providers while maintaining a secure and user-friendly payment experience.
Cloud Infrastructure
The entire platform is hosted on a cloud-based infrastructure optimized for performance, scalability, and security. Unthinkable leveraged cloud services to support HIPAA-aligned workloads, incorporating recommended configurations such as role-based access control, data encryption, audit logging, monitoring, and secure data retention. This cloud-native setup ensures high availability, regulatory compliance, and reliable performance across all user interactions.
