The client leads the nation in integrated eye care, managing a vast network of over 60 clinics across California. Renowned for clinical excellence, their team of ophthalmologists and optometrists delivers specialized treatments ranging from LASIK and retinal care to advanced oculoplastic procedures, setting a high standard for patient experience and service quality.
Business Situation
The eye care industry has seen rapid expansion, with growing patient volumes and increasing demand for specialized services. As the largest integrated ophthalmic network in the U.S., the client faced mounting operational challenges managing hundreds of daily patient visits.
Each location relied on a third-party EHR system to handle patient records, appointments, billing, telehealth, and insurance claims. However, as the network scaled, the system struggled to keep up with complex workflows. To add to it, recurring licensing fees became a significant financial burden for them.
Recognizing the need for a more agile and cost-effective solution, the client envisioned a custom-built EHR tailored to the nuances of ophthalmology.
To address these challenges, the client partnered with Unthinkable to build a purpose-led platform that aligned tightly with their clinical workflows and operational needs. The solution replaced third-party systems with a unified infrastructure, driving measurable gains in efficiency and reducing long-term costs. The Solution
To kick off the project, Unthinkable conducted a structured Discover & Frame (D&F) workshop to align stakeholders, clarify goals, and define the technical and functional blueprint for the solution. This helped in mapping the scope, timeline, and budget for a scalable EHR platform tailored to ophthalmology practices.
Core system architecture
Following the workshop, Unthinkable began development of a robust, web-based EHR system built on Angular and PostgreSQL. The architecture was designed to support six distinct user roles like front desk staff, call center agents, patients, doctors, technicians, and finance teams each with customized access and functionality.
In addition to resolving legacy system issues, the team implemented new features across modules and integrated APIs for insurance data exchange and third-party services, ensuring smooth performance and interoperability across networks.

Front desk scheduler module
The front desk module was designed to give staff a clear, real-time overview of daily operations. It displayed scheduled and active appointments alongside the patient’s financial payment summaries, including co-pays, deductibles, and outstanding balances. While the staff could monitor appointment progress, when a patient checked in, consulted a physician, or checked out, ensuring smooth coordination throughout the clinic.
Additionally, the system allowed front desk teams to generate comprehensive superbills. These included patient demographics, clinical notes, recommended follow-ups, authorization details, procedures performed, and appointment metadata, streamlining documentation and billing accuracy.

Appointment management system
The call center received advanced scheduling capabilities considering multiple factors like service type, patient zip code, clinic proximity radius, doctor preferences, and insurance coverage. This intelligent matching system optimized appointment scheduling across networks.

Billing and claims processing
The billing module enabled users to access a comprehensive list of patients with generated bills and seamlessly add new encounters. Each record captured essential details such as patient name, service date, exam type, insurance profile, provider, clinic location, authorization numbers, and billing modifiers ensuring accuracy and efficiency in financial documentation and claims processing

Insurance integration & claims management
Bills were automatically moved to the claims queue and submitted to clearinghouses. Approved claims showed up in the remittance section, while rejected or delayed ones were handled through a dedicated insurance module making the entire process faster and easier to manage.

Reporting dashboards
Unthinkable built custom dashboards for admins, doctors, and clinics. Admins could track revenue, claims, cancellations, new patient registrations, and feedback trends. Doctors had access to real-time data on patient loads, visit history, appointment schedules, and patient ratios helping them stay organized and improve care delivery.
Secure data architecture & compliance
To ensure complete data security and regulatory alignment, the system was built with full HIPAA compliance at its core, securing sensitive patient information across treatment, billing, and operational workflows. Unthinkable integrated specialized third-party services to support seamless mapping between diagnosis codes (ICD) and procedure codes (CPT), enabling accurate and justified insurance claims.
On the backend, a smart rule engine was implemented to verify code combinations using NCCI PTP (Procedure-to-Procedure) edits. This mechanism automatically flagged irregular billing codes during claim generation, significantly reducing rejections and speeding up reimbursement cycles.

The Impact
The custom EHR system is now live across all clinics, handling everything from patient care to admin tasks. Unthinkable made this rollout smooth by offering full deployment support and training for staff. As a result, productivity jumped by 60%, and the switch to a one-time investment helped cut EHR costs by 80%, eliminating the need for expensive third-party licenses.
In addition, Unthinkable is building a patient portal that’s nearly ready. It will let patients book appointments, view medical history, access clinical notes, track medications, and check insurance claims all in one place.
