SOAP Health is focused on improving healthcare by helping doctors catch serious illnesses like cancer and heart disease early. Their platform uses smart tools like an interactive interviewer and an AI-powered chatbot called Genie to collect patient information and create detailed SOAP notes (Subjective, Objective, Assessment, and Plan). The goal is to make it easier for patients and doctors to share information quickly and accurately, leading to faster and better diagnoses.
Business Situation
Early detection is crucial for saving lives from serious illnesses like cancer, heart disease, and type 2 diabetes. Studies show that 15% of medical visits result in diagnostic errors, causing around 400,000 deaths and another 400,000 disabilities each year, especially among patients with heart conditions and early-stage cancers.
SOAP Health runs SOAP Hub, an EHR platform that gathers patient data using a detailed interviewer module with 20+ questions across multiple screens. However, the system lacked interactive features, making it hard to keep patients engaged and collect complete medical histories. SOAP Health also wanted to add voice commands to its AI Genie chatbot, so users could speak instead of typing.
To solve these challenges, SOAP Health partnered with Unthinkable Solutions as experts in healthcare tech and app modernization. The goal was to upgrade the interviewer module and enhance the AI Genie with voice and interactive features for a smoother, smarter user experience.

The Solution
Unthinkable looked into SOAP Health’s challenges and came up with a clear plan to improve the platform. The goal was to make it easier for patients to interact and help healthcare providers work more efficiently.
Technology stack implementation
To build the solution, the team used Remix and React to create fast, responsive user interfaces that could handle complex patient interactions across 200+ screens.
Node.js with TypeScript was used at the backend for maintaining server-side logic, while Firebase enabled real-time data syncing between users and providers.
Tailwind CSS was used for consistent, mobile-friendly design, and Prisma provided secure and efficient access to the medical database.
The platform was deployed on Google Cloud using Cloud Functions and ExpressJS, offering a reliable and scalable infrastructure. To enhance user engagement, BabylonJS was integrated to deliver rich, interactive visual experiences within the application.

Voice user interface integration
SOAP Health wanted to make its Interviewer module easier and more engaging for users. To support this goal, Unthinkable integrated voice input across nearly 200 screens, allowing patients to speak their responses instead of typing. This made the experience smoother and more natural.
While implementing the feature, the team also fixed several bugs and improved system performance through regular testing and updates. These efforts helped ensure the module worked reliably and delivered a better experience to both patients and doctors.

AI Genie chatbot redesign
The original AI Genie chatbot on the SOAP Hub platform helped guide patients through health questionnaires, but its small size on the screen made it easy to miss.This limited its usefulness and caused patients to miss out on helpful prompts and support.
By redesigning the chatbot with a larger, more prominent design and integrating it seamlessly into the screen layout, the platform made it easier for patients to follow prompts, stay engaged, and receive the support they needed throughout the questionnaire process.
Voice command integration in AI Genie
The team redesigned it with a full-sized avatar that stood out clearly on the screen. This larger presence allowed the assistant to use expressive gestures, helping patients better understand instructions during the data entry process.
Now, voice commands were also integrated into the chatbot with a newly added microphone feature. This upgrade enabled patients to speak directly to the AI Genie, making interactions smoother and more natural, and greatly improving the overall user experience.

Iterative bug resolution and quality assurance
As the SOAP Hub platform continued to develop, frequent updates and code changes occasionally introduced bugs that disrupted both system performance and the patient experience.
To tackle this, the team at Unthinkable performed consistent testing that entailed quick resolution cycles which helped in addressing most of the problems efficiently, helping maintain a stable and smooth-running EHR platform.

The Impact
Unthinkable added speech-to-text functionality in over 200 screens in the SOAP Hub platform, making it easier for patients to speak their responses instead of typing. This voice integration made the data collection process faster and more user-friendly.
For healthcare providers, the updated platform helped cut down documentation work by 66%, giving them more time to focus on patient care. With AI features in place, doctors saved around 2 hours each day, allowing them to see more patients and improve service quality.
